Johns Hopkins Direct Primary Care (DPC) Plan

This plan option offers the same plan design as the PPO plan. There is no cost for an office visit with a DPC physician. You’ll have access to the EHP network and the preferred network, which includes Cigna PPO physicians and providers. In addition, employees who enroll in this plan will receive a $240 pre-funded lifestyle account, administered by Forma, to spend on certain lifestyle needs, such as gym memberships, fitness classes and meditation app subscriptions. 

This plan offers you and your adult dependents (18 years or older) 24/7, convenient access to select primary care services with extended office hours, longer appointment times and virtual services, such as video visits and MyChart messages.

As part of this benefit, select primary care services are available for no additional charge, including:

  • Primary care office or video visits, with no co-pay*
  • 1 annual physical
  • 1 flu vaccine
  • 6 rapid tests (any combination of flu, streptococcus, urine, pregnancy and A1C)
  • 1 electrocardiogram (EKG)
  • Unlimited telephone consultation
  • Unlimited direct access through phone and MyChart portal
  • Unlimited blood draws and urine collections*
  • Unlimited prior authorizations of medications
  • Robust care coordination

* Costs may be incurred for ancillary services conducted during the visit and for lab work.

Note: Enrollment space in the DPC PPO Plan is limited.

Forma Lifestyle Account

As mentioned above, employees who enroll in the DPC PPO Plan will receive $240 in funds to spend on certain health and wellness lifestyle needs. You will have two options to receive the $240 in funds: you may purchase items via the Forma Store or you may submit receipts for eligible purchases. Once a purchase is made, you will receive payment up to $240 via your paycheck or several paychecks until you reach $240 limit. Please note that you may only submit expenses that were incurred as of January 1, 2024 and it may take several paychecks to be paid out to you.

Understanding Available Provider Networks

Preferred Network

How much you pay for care depends on the services you need and where you go. For most covered services from Preferred Network providers and facilities, both plans pay 90% of the costs and you pay 10% (after any deductible). This is often the most affordable provider choice.

EHP Network

You also have access to the EHP Network, where 80% of the cost is covered, while you pay 20%. Fees vary among providers, so sometimes a low cost EHP network provider is the most affordable choice.

Cigna Network

In addition to the Preferred Network and EHP Network, you’ll have access to Cigna’s extended network of over one million nationwide in-network providers beyond those directly contracted in the EHP Network. Cigna’s PPO network providers in all U.S. states, are available to all EHP members. Cigna PPO network services are covered as in-network benefits.

Added Protection

If you want some added protection, consider enrolling in one or both of the following supplemental health care plans:

  • Accident Insurance: Pays a cash benefit directly to you for accidental injuries. You can use the money to help pay for some of the costs — both medical and non-medical related to your accident.
  • Critical Illness Insurance: Pays a lump-sum benefit upon the diagnosis of each covered illness.
  • Hospital Indemnity Insurance: Pays cash benefits that help pay for some of the costs — medical and non-medical — associated with a covered hospital stay due to a sickness, pregnancy or accidental injury.
